These forms are to help you convey useful information to first responders and doctors about your loved one's SMI history.


INSTRUCTIONS and best practices.

  1. Download. 
  2. Print to your personal printer.
  3. Fill out the printed form with a dark pen. (a) This is your original. (b) keep the original for your personal files. (b.1) scan and save on your personal computer.
  4. Copy the original. (a) make multiple copies to hand out to (a.1) all first responders; (a.1.1) police, (a.1.2) fire, and (a.1.3) crisis team. (a.2) clinics. (a.3) hospitals. (a.4) jail. (a.5) attorney. (a.6) an emergency contact.
  5. Don't expect people listed in #4 to keep their copy; best practice is to have copies to give out again and again. 
  6. Your loved one might not be prepared to read the content and answers given about them. Read about Anosognosia. 
  7. Plan on updating your original every year on your loved-ones birthdate, or as needed after a change in condition.
